Failure and Incident Investigation Essentials

Failure and Incident Investigation Essentials One Day Course

Objectives

This seminar is to provide an increased awareness of essential failure and incident investigation skills and tools.

Benefits of Training with Acuren

Acuren’s expert instructors provide many years of combined hands-on field-based experience relevant to the design, fabrication, and operation of pipelines, facilities, and pressure equipment. Their experience includes specializations in integrity management, failure investigation/analysis, material sciences, standards, jurisdictional requirements, industry best practices, industry code development, and competency programs. As a result, seminars go beyond a basic awareness of the codes of construction—attendees also increase their competencies in technical and practical areas.

Topics

The probability is high that individuals working in roles relating to integrity, maintenance, or operations will be involved in several failure or incident investigations during their careers. However, not all organizations have experienced teams dedicated to these types of investigations. As a result, other roles are often required to support or even lead investigations. In those situations, prior training and awareness will reduce the associated stress and increase the overall effectiveness of the process. This one-day seminar will use case studies and interactive discussions to increase the audience’s confidence and awareness of key concepts and tools associated with failure and incident investigations including:
  • Language of investigations
  • Causal factors
  • Report writing
  • Anatomy of an incident
  • Evidence collection and preservation
  • Regulatory requirements
  • Stakeholders and investigation teams
  • Role of lab work (e.g., material testing, metallography, fractography)
  • Corrective Actions
  • Investigation techniques
  • Safety and site controls
  • Witness statements
Attendees will leave the seminar with an increased level of confidence in their ability to support or lead investigations at their work site. Course presentation materials will be provided to all participants.
